Nara, Miyajima, Kobe, Hiroshima for sure. We did the day trip to see Mt. Fuji but it was cloudy and raining and went to onsen instead.

From the peace park near the river there are fast boats that can take you out to Miyajima, last time I was there it was about 2000 yen per person. it get's you there in about 30 minutes and you feel like James Bond while standing on the aft deck.

Koya-San
Also Miyajima for oysters
In Osaka go to Fugestu for yakisoba and okonomiyaki and eat at Shissen Ramen. Also have takoyaki from a street vendor. Try Hokkaido pan and melon pan from 7-11 and if it’s in season ichigodaifuku. Go to the basement of any department store and sample the goodies there. Look for my very favourite sweet on the planet from Ise prefecture called Akafuku!

Miyajima island definitely. It is a day trip from Osaka, but if you stay in Hiroshima, you would have more time to spend there
